This brochure website is designed to provide general information. It does not include a medical intake form and should not be used to send symptoms, diagnoses, medications, treatment history, or other protected health information.
Secure Systems
Scheduling, intake, patient documents, and protected health information will be handled through approved external systems such as OptiMantra and Spruce. Those services have their own privacy terms and registration requirements.
